ICL recovery timeline: what to expect after lens surgery
How long does ICL recovery take?
Most people see clearly within a day of ICL surgery and return to light activities within a few days; the eye settles fully over one to three months. Clinics watch eye pressure closely in the first 24 hours.
An implantable collamer lens (ICL) sits inside the eye, behind the iris, inserted through an incision of about 3 mm. The cornea is not reshaped, so the early checks focus on pressure, inflammation and lens position. The STAAR Surgical EVO ICL has a central port that removes the need for the laser iridotomy older lenses required. See ICL surgery in Korea.
What does the ICL recovery timeline look like?
| Time after surgery | What you usually notice | What clinics typically do |
|---|---|---|
| 1–6 hours | Blur, watering, mild ache | Measure eye pressure before you leave |
| Day 1 | Vision often clear; halos common | Check pressure, inflammation and vault |
| Days 2–7 | Mild grittiness; night halos | Steroid and antibiotic drops; shield at night |
| Week 1 | Most daily tasks comfortable | Follow-up; many surgeons clear flying |
| Weeks 2–4 | Vision stable for most people | Drops tapered; gradual return to exercise |
| Months 1–3 | Final visual result | Review of pressure, lens position, vision |
Why is eye pressure checked so soon?
Because a short-lived rise is common and easy to treat early. In the three-year US FDA study of EVO/EVO+ ICL (629 eyes), 19.9% of eyes had a transient pressure rise 1–6 hours after surgery, linked to retained viscoelastic gel. A later steroid-related rise occurred in 2.4%, and no pupillary block was reported. Severe pain, nausea or rainbow haloes in the first days need urgent attention.
How good is vision after ICL?
Results are generally strong but not guaranteed. In the same study, 85.8% of eyes saw 20/20 or better without glasses at three years and 99.5% saw 20/40 or better. STAAR lists EVO ICL for myopia of about -3.00 to -20.00 D, with up to 4.00 D of astigmatism via a toric ICL, in adults 21–45. Compare options in ICL vs LASIK.
What should I avoid after ICL surgery?
- No rubbing or pressing on the eye; wear the shield to sleep
- Keep tap water, soap and shampoo out of the eye
- No swimming, saunas or hot tubs until cleared, usually several weeks
- No heavy lifting or strenuous exercise for at least the first week
- Use steroid and antibiotic drops exactly as prescribed
When can I fly after ICL?
Many surgeons allow a short flight within a few days to a week, once checks show stable pressure and lens position. For a long-haul flight from Seoul, aim to complete the one-week check. See flying after eye surgery.
What are the risks?
STAAR's safety information lists raised pressure and glaucoma, cataract, corneal endothelial cell loss, inflammation, retinal detachment, lens rotation or dislocation needing further surgery, and infection including endophthalmitis. In the FDA data, cataract events affected under 1% of eyes, 0.32% needed a lens exchange for high vault, and mean endothelial cell loss was 6.7% at three years. Yearly eye checks are recommended; ask your clinic for a record of the lens model, size and power.
How long should I stay in Seoul?
About a week: dilated pre-op exam, surgery, day-one check and ideally a one-week review. Some clinics do both eyes the same day, others on separate days. Compare EVO ICL clinics in Gangnam. This is general information; an ophthalmologist's full exam decides whether ICL suits you.
What happens on ICL surgery day?
ICL surgery itself is short, but expect to spend several hours at the clinic because your pressure is checked before you leave.
Your pupil is dilated, numbing drops are applied, and the folded lens is inserted through the small incision and unfolds behind the iris. Most people are awake and feel pressure rather than pain. Afterwards you rest at the clinic while staff check eye pressure, usually a few hours later. Have someone accompany you back to your hotel, and do not plan anything for the rest of the day.
What is vault, and why is it checked?
Vault is the gap between the back of the ICL and your natural lens. It is checked at the day-one visit and later reviews because a vault that is too low or too high can cause problems.
| Vault | Possible concern | Usual response |
|---|---|---|
| Within the expected range | None | Routine follow-up |
| Too low | Higher long-term risk of cataract | Closer monitoring; lens exchange if needed |
| Too high | Narrowing of the drainage angle and raised pressure | Monitoring; lens exchange if needed |
In the US FDA EVO study, 0.32% of eyes needed a lens exchange because of angle narrowing with high vault. Lens sizing is based on your pre-op measurements, which is why a thorough pre-op eye exam matters.
What follow-up do I need once I am home?
Plan on regular eye checks for as long as the lens is in the eye, typically at about one and three months and then yearly. The checks look at eye pressure, lens position, the cornea's endothelial cells and early signs of cataract.
Before leaving Seoul, ask for a written summary with the lens model, size and power, your surgery date and your drop schedule. An ICL can be removed or exchanged if needed, which is one reason some people choose it over laser surgery. Compare long-term trade-offs in ICL vs SMILE and costs in cost of EVO ICL in Korea.

FAQ
Frequently asked questions
Is ICL recovery painful?
Most people feel mild aching, grittiness or watering for the first day rather than real pain. Severe pain, especially with nausea or headache, can mean high eye pressure and needs urgent attention.
How soon can I see after ICL?
Many people notice clear vision within hours, and most see well by the next morning. Some blur, glare and halos are normal while the eye settles.
When can I return to work after ICL surgery?
Desk workers often return within a few days to a week, after the early checks. Physically demanding or dusty jobs usually need longer; ask your surgeon.
Can I exercise after ICL?
Light walking is usually fine within days. Strenuous exercise, heavy lifting and contact sports typically wait at least one to two weeks, and swimming several weeks, depending on your surgeon.
Are both eyes done on the same day?
Some Seoul clinics treat both eyes on the same day, while others leave a day or more between eyes. Ask in advance, as it changes how long you need to stay.
