ICL vs SMILE: Lens Implant or Flapless Laser?
How do ICL and SMILE differ?
SMILE corrects vision by removing a thin disc of corneal tissue with a femtosecond laser; ICL corrects it by implanting a thin lens inside the eye, leaving the cornea intact.
In SMILE, a ZEISS VisuMax laser shapes a lenticule inside the cornea, which the surgeon removes through a 2–4 mm incision. In ICL surgery, a lens such as the STAAR EVO Visian ICL is folded, inserted through a small incision and placed behind the iris. Read the SMILE guide and the ICL guide for each.
ICL vs SMILE side by side
The right column for you depends on your exam, especially corneal thickness and anterior chamber depth.
| Factor | SMILE | ICL (EVO Visian) |
|---|---|---|
| Where it works | Inside the cornea | Inside the eye, behind the iris |
| Tissue removed | Yes (lenticule) | No |
| Prescription range | Mild to moderate myopia and astigmatism; capped by corneal thickness | FDA indication -3.0 to -20.0 D; toric for 1.0–4.0 D astigmatism |
| Thin corneas | Often limits candidacy | Usually not a barrier |
| Reversible | No | Yes |
| Main specific risks | Under/over-correction, haze or interface issues, rare ectasia | Raised eye pressure, cataract, endothelial cell loss, infection |
| Price in Seoul | ₩2.2M–₩3.2M both eyes (≈$1,600–$2,400) | ₩4M–₩6.5M both eyes (≈$2,900–$4,700) |
Which is better for high myopia?
For high myopia, ICL is usually the safer fit because it does not remove corneal tissue.
Every laser procedure, SMILE included, removes more tissue the stronger the correction. When too little cornea would remain, the risk of ectasia rises. EVO and EVO+ ICLs in the US are indicated from -3.0 to -20.0 D with a stable prescription. Understand the thresholds in corneal thickness for LASIK.
Which is better if I have dry eyes?
Both are gentler on the ocular surface than flap LASIK, and the choice is usually made on prescription and anatomy instead.
SMILE avoids a large flap, so fewer corneal nerves are cut; a 2016 meta-analysis found better corneal sensation and fewer early dry eye symptoms after SMILE than after femtosecond LASIK. ICL leaves corneal nerves almost untouched. Existing dry eye should be treated before any surgery; see dry eye after LASIK.
Which is better for astigmatism?
Both can treat astigmatism, so the deciding factors are usually how much myopia you have alongside it and how thick your cornea is.
SMILE corrects astigmatism within the same lenticule; the FDA approval announced for the VISUMAX 800 with SMILE pro software covers nearsightedness with or without astigmatism, and ZEISS's OcuLign aid suggests a correction for eye rotation, which matters most when astigmatism is being treated. For ICL, a toric version is used: the US EVO ICL label covers 1.0–4.0 D of cylinder together with -3.0 to -20.0 D of myopia. If you have high myopia plus astigmatism, a toric ICL is often discussed; see toric ICL in Korea and astigmatism correction in Korea.
Is ICL riskier than SMILE?
ICL is intraocular surgery, so its rare risks are different and can be more serious.
The FDA lists phakic lens risks including raised eye pressure, cataract, endothelial cell loss, retinal detachment and infection, plus the possibility that the lens must be repositioned or removed. SMILE's risks sit in the cornea: residual refractive error, interface haze, and rarely ectasia. Neither guarantees perfect vision.
How does recovery compare?
Both usually allow functional vision within a day or two.
SMILE patients often return to desk work in a couple of days, with sharpness improving over weeks. ICL patients use drops and need eye-pressure checks in the first days, so plan to stay in Seoul for follow-up before flying. See the SMILE recovery timeline and ICL recovery timeline.
What do ICL and SMILE cost in Seoul?
For both eyes, ICL typically costs roughly twice as much as SMILE in Seoul.
| Procedure | Seoul price (KRW) | Approx. USD |
|---|---|---|
| SMILE (VisuMax 500) | ₩2.2M–₩3.2M, both eyes | $1,600–$2,400, both eyes |
| SMILE Pro (VisuMax 800) | About ₩3M–₩5M, both eyes | $2,200–$3,700, both eyes |
| ICL | ₩4M–₩6.5M, both eyes | $2,900–$4,700, both eyes |
Prices were checked in 2026, at roughly ₩1,350–1,400 per dollar. Toric ICLs for astigmatism cost more, about ₩5M–₩7.5M for both eyes, so ask for a quote that names the lens type. See SMILE cost, ICL cost and toric ICL cost in Korea.
Who should choose which?
Let your measurements lead; if both are safe, weigh cost against reversibility.
- SMILE may suit you if you have mild-to-moderate myopia, enough corneal thickness, an active lifestyle and want a lower price.
- ICL may suit you if you have high myopia, thin or irregular-risk corneas, want a removable option and have adequate anterior chamber depth.
- Wait if your prescription changed in the last year or you have untreated eye disease.
What should I ask the clinic about ICL vs SMILE?
When a clinic recommends one over the other, ask it to show you the measurements behind the advice.
- How much tissue would SMILE remove for my prescription, and what residual thickness would remain?
- What are my anterior chamber depth and endothelial cell count? (The US EVO ICL label requires a chamber depth of at least 3.0 mm.)
- Which laser would be used for SMILE, VisuMax 500 or VISUMAX 800?
- Which ICL model and size would I receive, and do I need a toric lens?
- How long should I stay in Seoul after each procedure before flying?
Always get a full exam and an ophthalmologist's opinion; see questions to ask your LASIK surgeon.

FAQ
Frequently asked questions
Is ICL or SMILE better for -8 D myopia?
Many surgeons favour ICL at that level, especially if the cornea is average or thin, because SMILE would remove a lot of tissue. Some thick corneas can still be treated with SMILE; your exam decides.
Can I have SMILE after ICL, or ICL after SMILE?
Yes, in some cases. A small residual error after ICL can sometimes be fine-tuned with laser surgery, and ICL can be considered after laser surgery if enough anatomy allows. Your surgeon will advise.
Which has less dry eye, ICL or SMILE?
Both are lower-risk than flap LASIK. ICL disturbs corneal nerves least, while SMILE's small incision spares many nerves compared with LASIK.
How much does ICL cost compared with SMILE in Korea?
ICL costs ₩4M–₩6.5M for both eyes in Seoul, while SMILE costs ₩2.2M–₩3.2M for both eyes, so ICL usually costs roughly twice as much.
What is the age range for ICL?
In the US, the FDA indication for EVO and EVO+ ICLs was expanded in 2026 from ages 21–45 to 21–60. Clinics in Korea set their own criteria after an exam.
