Recovery guide

SMILE recovery timeline: day 1 to month 3

After SMILE, most people see clearly enough for daily life by the next day, and ZEISS says vision normally stabilises within a few weeks. Because SMILE uses a 2–4 mm incision instead of a flap, studies generally report less early dry eye than LASIK, though recovery varies by eye.

SMILE recovery timeline: day 1 to month 3

How long is SMILE recovery?

Most people see well enough for daily life the day after SMILE, and ZEISS, maker of the VisuMax laser, says vision should normally stabilise within a few weeks. Fine sharpness can keep improving for up to about three months.

SMILE (small incision lenticule extraction) cuts a thin disc of tissue inside the cornea and removes it through a side incision of roughly 2–4 mm, with no LASIK-style flap. SMILE Pro on the VisuMax 800 shortens laser time, but recovery is broadly similar. See the full SMILE guide.

What does a typical SMILE recovery look like?

Time after surgery What you usually notice What to do
0–4 hours Slight blur, watering, scratchiness Get a ride back; rest with eyes closed
Day 1 Vision often clear enough for most tasks, sometimes slightly hazier than after LASIK Next-day check; start prescribed drops
Days 2–7 Mild fluctuation, light sensitivity, halos Screen breaks, sunglasses outdoors, no rubbing
Weeks 1–4 Sharpness improving; dryness usually mild Return to routines as cleared
Months 1–3 Vision settles; night halos fading Review visit; swimming and sport when cleared

What happens on surgery day and the day after?

Expect blurry vision for a few hours and do not drive. ZEISS says visual recovery can come as soon as the next day, and many patients resume daily activities then. Some people find day one slightly hazier than after LASIK, with SMILE catching up within weeks. If you need crisp vision the next day, tell your surgeon; see SMILE vs LASIK.

Is dry eye less of a problem after SMILE?

Generally yes, in the first months. A 2019 review in the Asia-Pacific Journal of Ophthalmology found most studies and several meta-analyses favoured SMILE over femtosecond LASIK for dry eye symptoms and corneal sensation, because the small incision cuts fewer nerves. In one cited study, 80% of SMILE patients versus 57% of LASIK patients no longer needed artificial tears at six months. The authors noted most studies were small and non-randomised, so the benefit is likely but not guaranteed. Treatments are in dry eye after LASIK.

What should I avoid after SMILE?

Avoid rubbing your eyes and getting water in them while the incision seals. ZEISS says showering is fine if water stays out of your eyes, and recommends screen breaks and UV sunglasses in the first days. It gives no fixed timeframe for sport or swimming: your surgeon decides. Use antibiotic, anti-inflammatory and lubricating drops exactly as prescribed.

When can I fly home after SMILE?

Most surgeons allow flying after a normal next-day check. Pressure does not affect the cornea, but cabin air is dry, so use drops in flight. Compare procedures in flying after laser eye surgery.

What are the warning signs after SMILE?

Call your clinic if pain increases, the eye becomes very red or sticky, or vision drops suddenly. Infection and inflammation are uncommon but need prompt treatment. SMILE can also leave under- or over-correction, glare or halos; retreatment options are in LASIK enhancement in Korea. This is general information; a full exam and your ophthalmologist's advice decide whether SMILE suits you. Clinics are listed in top SMILE clinics in Seoul.

How does SMILE recovery compare with LASIK and PRK?

SMILE sits between LASIK and PRK: early vision is close to LASIK, discomfort is mild, and there is no flap or surface wound to heal.

Factor SMILE LASIK PRK / LASEK
Opening in the cornea About 2–4 mm incision Flap about 8–9 mm across Surface layer removed
Usable vision Usually next day Often same evening or next day About days 4–7
Early dry eye Generally lower Common Common
Trauma risk after healing No flap to dislodge Small flap risk No flap

What happens at the next-day check?

The next-day check confirms that the incision has sealed, the cornea is clear and there is no sign of infection or inflammation. You will have a vision test and a slit-lamp examination, and the clinic will go through your drop schedule.

Some patients notice slightly lower sharpness at this visit than they expected. This is common in the first days after SMILE and usually improves over the following weeks as the cornea settles. Ask the clinic when your next review is due, and whether a one-week or one-month check can be done by an ophthalmologist at home.

Bring a list of the drops you were given and when to use them, plus any questions about screens, exercise or makeup. Clinics generally taper anti-inflammatory drops over the first weeks while lubricating drops continue as long as you need them. If one eye feels noticeably different from the other at this visit, say so: small differences are common, but the clinic should record them.

How long should I stay in Seoul after SMILE?

Plan on at least three to four days: one for the full pre-operative exam, one for surgery, and one for the next-day check, plus a spare day in case anything needs a second look. A week gives more margin and lets you attend a one-week review.

Book your flight home only after your surgery date is confirmed, because the exam may show another procedure suits you better. Our 3-day LASIK trip itinerary also works for SMILE, and costs are covered in SMILE cost in Korea vs the USA.

FAQ

Frequently asked questions

Is SMILE recovery faster than LASIK?

Not always on day one: LASIK often gives slightly sharper vision in the first days. SMILE tends to have less dry eye in the following months, and having no flap removes the risk of flap problems after an injury.

How soon can I go back to work after SMILE?

Many people return to desk work within one to three days, once the next-day check is normal. Jobs involving dust, chemicals or heavy physical work may need longer.

Can I use my phone after SMILE?

Yes, in moderation. Staring at a screen reduces blinking, which worsens dryness, so take frequent breaks and use lubricating drops in the first weeks.

When can I wear eye makeup after SMILE?

ZEISS notes many patients resume routine care, including makeup, shortly after surgery, but timing varies by clinic. Ask your surgeon before using eye makeup.

Does SMILE hurt during recovery?

Most people describe mild scratchiness or watering for a few hours rather than pain. Significant or increasing pain is not normal and should be reported to your clinic.
