Refractive Lens Exchange vs ICL: Which Is Better? (2026)

ICL adds a removable lens and keeps your natural lens, so it usually suits people under about 45 with high myopia. Refractive lens exchange (RLE) replaces the natural lens, treating presbyopia and preventing cataract, so it suits people over 45. In Seoul, ICL costs ₩4M–₩6.5M for both eyes; RLE ₩1.5M–₩6.5M per eye.

Refractive Lens Exchange vs ICL: Which Is Better? (2026)

Is RLE or ICL better?

Age is usually the deciding factor. ICL is generally better for younger adults with high myopia who still focus up close naturally. RLE is generally better for people over about 45 who already need reading glasses or have early lens changes.

Both are lens-based procedures for prescriptions or corneas that are not suitable for laser surgery. The difference is what happens to your own crystalline lens. An ICL leaves it in place and adds a thin lens in front of it. Refractive lens exchange removes it, exactly as in cataract surgery, and replaces it with an artificial intraocular lens (IOL). That single difference changes who benefits, which risks matter and how reversible the result is.

How do ICL and RLE work?

ICL is additive and reversible; RLE is a lens replacement and is effectively permanent.

With ICL, the surgeon inserts a foldable collamer lens, such as the STAAR Surgical EVO ICL, through a small incision and positions it behind the iris. Your natural lens keeps focusing as before until presbyopia arrives. If needed, the ICL can be removed or exchanged later, and when you eventually develop cataract it is taken out during cataract surgery.

With RLE, the surgeon breaks up and removes your natural lens using ultrasound (phacoemulsification) and implants an IOL. You can choose a monofocal or multifocal lens, a toric lens for astigmatism, or an EDOF lens. Because the natural lens is gone, you can never develop a cataract in that eye, but you also lose any remaining natural focusing ability.

RLE vs ICL side by side

Factor ICL (EVO) Refractive lens exchange
Natural lens Kept Removed and replaced
Typical age About 21–45 (US label 21–60) Usually 45+
Best for High myopia, thin corneas Presbyopia, high hyperopia, early lens changes
Near vision without glasses Natural until presbyopia Only with multifocal, EDOF or monovision
Reversible Yes, lens removable IOL exchange possible, natural lens gone
Future cataract Possible; small added risk Cannot occur
Eye anatomy needed Anterior chamber ≥3.0 mm Fewer chamber limits
Retinal detachment Low Higher, especially in high myopes
Seoul price (2026) ₩4M–₩6.5M both eyes (≈$2,900–$4,700) About ₩1.5M–₩6.5M per eye by lens
Typical stay About 5–7 days About 5–7 days

Note that ICL prices are for both eyes and RLE prices are per eye. Dollar figures are approximate at ₩1,350–1,400 per dollar; prices were checked in 2026. See RLE cost in Korea and ICL cost in Korea.

What about retinal detachment risk?

Retinal detachment is the main reason surgeons are cautious about RLE in younger, very short-sighted people.

Highly myopic eyes already have a higher baseline risk of retinal detachment, and removing the natural lens raises it further. A 2005 British Journal of Ophthalmology series of 62 highly myopic eyes reported retinal detachment in 2 eyes (3.2%) after RLE, and the authors stressed careful patient selection. An AAO EyeNet roundtable cited a 1999 study reporting 8% detachment within seven years in high myopes having lens surgery, and one surgeon judged that risk unacceptable for elective lens exchange. All About Vision likewise notes that RLE is usually not recommended for myopia when LASIK, PRK or phakic lenses are options. For ICL, STAAR reported no pupillary block or pigment dispersion and 0.16% anterior subcapsular cataract in 629 eyes over three years.

Which is better for presbyopia?

RLE is the only one of the two that can treat presbyopia directly, by using a multifocal, EDOF or monovision lens plan.

An ICL corrects distance prescription only; once presbyopia starts in your 40s, you will need reading glasses as anyone else does. That is why surgeons often steer patients over about 45 to RLE, particularly if they are farsighted or have early lens clouding. Expanded US approval of EVO ICL to age 60 in 2026 gives older myopes another option, but the presbyopia trade-off remains. See presbyopia treatment in Korea and LASIK after 40.

What are surgery and recovery like?

Both are done under numbing drops, usually one eye at a time a day or more apart, and most people see well within a few days.

ICL surgery takes roughly 15–20 minutes per eye. Eye pressure is checked a few hours afterwards because a lens that sits too high (a high vault) can raise it. RLE is the same operation as modern cataract surgery and takes a similar time; vision may be blurry for a day or two, and multifocal lenses need weeks to months for the brain to adapt. Both need antibiotic and anti-inflammatory drops for several weeks, no eye rubbing and no swimming for at least a couple of weeks. Plan around a week in Seoul so both eyes can be checked before you fly, and keep your operation records for your eye doctor at home. See the ICL recovery timeline.

Who should choose which?

ICL often suits you if you:

  • Are under about 45 and still read comfortably without glasses
  • Have high myopia or thin corneas and enough anterior chamber depth
  • Want a reversible procedure that keeps your natural lens

RLE often suits you if you:

  • Are over about 45, already use reading glasses or have early cataract
  • Are farsighted, especially with a shallow anterior chamber
  • Want near and distance vision addressed in one operation

High myopes over 45 are the grey zone: discuss retinal risk, presbyopia goals and a dilated retinal exam with your surgeon. See options for high myopia.

FAQ

Frequently asked questions

Is ICL or lens replacement better after 45?

Often lens replacement, because it can address presbyopia and prevents cataract. EVO ICL is now US-labelled to age 60, so high myopes should discuss both options and retinal risk.

Can I have RLE if I am very nearsighted?

Sometimes, but high myopes have a higher retinal detachment risk after lens removal. Many surgeons prefer ICL for younger high myopes.

Is an ICL reversible?

Yes. An ICL can be removed or exchanged. RLE cannot be reversed in the same way because your natural lens has been removed.

How much does RLE cost in Korea compared with ICL?

In Seoul, ICL costs about ₩4M–₩6.5M for both eyes. RLE costs about ₩1.5M–₩6.5M per eye depending on the lens, checked in 2026.

Will I get cataracts after ICL?

Cataract can develop with age, and ICL adds a small risk. In STAAR's three-year FDA data, anterior subcapsular cataract occurred in 0.16% of eyes.
