Toric ICL vs LASIK for Astigmatism: Which Is Better? (2026)

LASIK usually suits moderate astigmatism with low to moderate myopia and healthy, thick enough corneas. Toric ICL suits astigmatism of 1–4 D with myopia from −3 to −20 D, or thin corneas, and is removable. In Seoul, LASIK costs ₩1.5M–₩2.5M and toric ICL about ₩5M–₩7.5M, both eyes.

Toric ICL vs LASIK for Astigmatism: Which Is Better? (2026)

Is toric ICL or LASIK better for astigmatism?

For moderate astigmatism with a low to moderate glasses prescription and healthy corneas, LASIK is usually simpler and cheaper. For astigmatism combined with high myopia, thin corneas or dry-eye concerns, a toric ICL is often the better option.

Astigmatism means the eye focuses light unevenly, usually because the cornea is shaped more like a rugby ball than a football. LASIK corrects it by removing tissue to reshape the cornea. A toric ICL corrects it optically: a lens with built-in cylinder sits behind the iris, aligned on a precise axis. The decision depends on how much astigmatism and myopia you have, your corneal thickness and shape, and the space inside your eye, all measured at a full pre-op exam. See also astigmatism.

Toric ICL vs LASIK side by side

Factor Toric ICL (EVO) LASIK
How it corrects astigmatism Toric lens implanted behind the iris Excimer laser reshapes the cornea under a flap
US-labelled range Myopia −3 to −20 D with 1–4 D cylinder Varies by laser; e.g. up to −5 D cylinder (Visx iDesign) or −3 D (Contoura)
Corneal tissue removed None Yes; more for higher corrections
Thin corneas Often suitable Often unsuitable
Reversible Yes, lens can be removed or exchanged No
Specific risks Lens rotation off axis, eye pressure, cataract (small) Dry eye, flap issues, ectasia (rare)
Needs Anterior chamber depth ≥3.0 mm, stable refraction Enough corneal thickness, regular corneal shape
Seoul price, both eyes (2026) About ₩5M–₩7.5M (≈$3,600–$5,500) ₩1.5M–₩2.5M (≈$1,100–$1,900)
Typical stay in Seoul About 5–7 days About 3–5 days

Prices were checked in 2026 at about ₩1,350–1,400 per dollar. A toric ICL typically costs about ₩1M more than a standard ICL; see the toric ICL cost guide and LASIK cost in Korea.

How much astigmatism can each correct?

Toric EVO ICL is US-labelled for 1–4 D of cylinder; LASIK limits depend on the laser and, above all, your cornea.

STAAR Surgical's 2026 US indication covers myopic astigmatism with a spherical equivalent of −3 to −20 D and 1.0–4.0 D of cylinder at the spectacle plane. For lasers, FDA submission data reviewed in 2017 showed labelled cylinder limits of up to −5 D for the Visx iDesign wavefront platform and up to −3 D for Alcon's Contoura topography-guided platform. In practice, the binding limit for LASIK is often corneal thickness: correcting high myopia plus astigmatism removes more tissue. Regular astigmatism is required; irregular astigmatism or keratoconus rules out routine LASIK and needs specialist assessment.

What are the specific risks?

Both are generally safe in suitable eyes, but the risks differ.

With a toric ICL, the lens must stay on its planned axis. If it rotates, astigmatism correction falls off and the surgeon may need to reposition it in a short procedure. The space between the ICL and your natural lens (the vault) and the eye pressure are checked after surgery. In STAAR's three-year US trial of 629 eyes, there was no pupillary block or pigment dispersion and anterior subcapsular cataract occurred in 0.16%. With LASIK, dry eye is common for weeks to months, night glare can occur, a hard blow can displace the flap, and rarely the cornea weakens (ectasia), which is why tomography screening matters. Under-correction of astigmatism can also leave residual blur that needs an enhancement. See LASIK side effects and the ICL recovery timeline.

What about SMILE or combined approaches?

SMILE is another laser option for astigmatism, and some high prescriptions are treated with a lens plus laser.

SMILE has been FDA-approved for myopic astigmatism since 2018 and avoids a flap. Topography-guided options such as Contoura Vision may suit astigmatism with mild corneal irregularity. For very high myopia with astigmatism beyond a single lens's range, some surgeons combine an ICL with a later laser touch-up; ask whether that applies to you. Compare with ICL vs LASIK and LASIK with astigmatism.

How does surgery day differ?

LASIK takes minutes for both eyes in one session; a toric ICL needs axis marking and is usually done one eye at a time.

For LASIK, numbing drops are used, the flap is cut and the laser treats both eyes in about 15–20 minutes in total; eye trackers help keep the astigmatism treatment aligned. For a toric ICL, the lens is ordered to your measurements in advance, which can take time, so clinics often schedule the exam well before surgery. On the day, the surgeon marks the eye's axis, inserts the folded lens through a small incision and rotates it into position. Eye pressure is checked a few hours later, and the axis is checked again at follow-ups. Ask your Seoul clinic whether the toric lens is in stock or must be ordered, as this affects how long you need to stay.

Who should choose which?

LASIK often suits you if you:

  • Have regular astigmatism with low to moderate myopia
  • Have healthy corneas thick enough for the full correction
  • Want the lower price and a shorter stay in Seoul

Toric ICL often suits you if you:

  • Have myopia of about −3 D or more with 1–4 D of astigmatism
  • Have thin corneas, dry-eye concerns or were told LASIK would remove too much tissue
  • Want a reversible option and have enough anterior chamber depth

Your ophthalmologist should make the final recommendation after measuring both your cornea and the inside of your eye.

FAQ

Frequently asked questions

Is toric ICL better than LASIK for astigmatism?

It depends on your eyes. LASIK is usually better for moderate astigmatism with healthy, thick corneas; toric ICL is often better with high myopia, thin corneas or dry eye.

How much astigmatism can a toric ICL correct?

The US label for toric EVO ICL covers 1.0–4.0 D of cylinder with −3 to −20 D of myopia. Ranges can differ in other countries.

Can a toric ICL rotate?

Yes, occasionally. If it rotates off axis, astigmatism correction is reduced and the surgeon can reposition it in a short procedure.

How much does toric ICL cost in Korea?

In Seoul, toric ICL costs about ₩5M–₩7.5M for both eyes, roughly ₩1M more than a standard ICL, based on 2026 checks.

Can LASIK fix high astigmatism?

LASIK can treat moderate to fairly high regular astigmatism, depending on the laser and your corneal thickness. Very high or irregular astigmatism may need other options.
