VisuMax 500 vs VisuMax 800: Which Is Better? (2026)

Both are ZEISS femtosecond lasers for SMILE. The VisuMax 800 fires at 2 MHz and cuts the lenticule in under 10 seconds, versus about 25–30 seconds at 500 kHz on the older VisuMax, and adds alignment aids. Results appear similar. In Seoul, SMILE Pro on the 800 costs ₩3M–₩5M (typically around ₩4M) vs ₩2.2M–₩3.2M for SMILE.

VisuMax 500 vs VisuMax 800: Which Is Better? (2026)

Is the VisuMax 800 better than the VisuMax 500?

The VisuMax 800 is faster and adds alignment tools, which can make the procedure easier for anxious patients and some astigmatism cases. Published experience so far suggests visual results are broadly similar to the VisuMax 500, so a skilled surgeon on either laser is more important than the model.

Both lasers come from ZEISS and perform the same core operation: a femtosecond laser cuts a thin lenticule inside the cornea, which the surgeon removes through a small incision with no flap. On the older VisuMax (500 kHz) this is marketed as SMILE; on the VisuMax 800 it is SMILE Pro. Both can also cut LASIK flaps. More than 8 million eyes had been treated with SMILE worldwide by early 2024, according to ZEISS.

VisuMax 500 vs VisuMax 800 side by side

Factor VisuMax 500 (SMILE) VisuMax 800 (SMILE Pro)
Laser pulse rate 500 kHz 2 MHz
Lenticule cutting time About 25–30 seconds Under 10 seconds (6.5 mm zone, myopia)
LASIK flap cut Yes About 5 seconds
Centration aid Manual CentraLign auto adjusts laser head before docking
Cyclotorsion (eye rotation) aid Manual marking OcuLign uses an iris image to suggest compensation
Re-treatment option Surface ablation or other methods CIRCLE converts the cap into a flap for LASIK re-treatment
Patient positioning Patient moved between laser and microscope Arm swings over; patient stays still
Seoul price, both eyes (2026) ₩2.2M–₩3.2M (≈$1,600–$2,400) ₩3M–₩5M, typically around ₩4M (≈$2,200–$3,700)

Prices were checked in 2026 at about ₩1,350–1,400 per dollar. See SMILE Pro cost in Korea and SMILE cost in Korea.

Does faster laser time matter?

It matters mostly for comfort and for the chance of suction loss, not for final vision.

During the laser, a curved contact glass holds the eye with gentle suction, and vision dims. Holding still for under 10 seconds is easier than for 25–30 seconds, especially for nervous patients. In a 2023 report in Ophthalmology Times Europe, surgeon Sheetal Brar described one suction loss in about 1,200 eyes (0.08%) on the VisuMax 800, compared with published rates of 0.5% in 4,000 eyes and a range of 0.17%–5.06% on the 500 kHz laser. Those figures come from different series and the author consults for ZEISS, so they are not a controlled comparison.

Do results differ between the two lasers?

Current evidence suggests similar visual results; the 800's advantages are mainly speed and workflow.

The same 2023 report described outcomes on the VisuMax 800 as comparable to the 500 kHz platform, based on the author's experience rather than a randomised trial. Treatment profiles and incisions are essentially the same, and both remove a lenticule through a small incision, so the benefits of SMILE over LASIK, such as fewer cut corneal nerves and no flap, apply to both. The tools that could matter for outcomes are the 800's automated centration and cyclotorsion support, which may help in higher astigmatism; ask your surgeon how they use them. For the procedure-level comparison, see SMILE vs SMILE Pro and SMILE vs LASIK.

Is SMILE Pro worth the extra cost?

For many people, standard SMILE on a well-maintained VisuMax 500 is excellent value; SMILE Pro is worth considering if you are anxious, have significant astigmatism or want the clinic's newest platform.

In Seoul, the premium for the VisuMax 800 is typically about ₩0.8M–₩1.8M for both eyes. Recovery is similar: most people see well within a day or two and return to desk work within a few days. If budget is tight, spend it on a careful exam, an experienced surgeon and good aftercare rather than the newest laser. Read the SMILE recovery timeline.

What should I ask a clinic about its laser?

Ask which laser will treat your eyes, and make sure it is written on your quote.

A clinic may own both platforms, and the names SMILE and SMILE Pro are not always used precisely in advertising. Useful questions:

  • Is my procedure on the VisuMax 800 (SMILE Pro) or the 500 kHz VisuMax (SMILE)?
  • How many SMILE procedures has my surgeon done on this laser?
  • Will you use automated centration or cyclotorsion compensation for my astigmatism?
  • If I need an enhancement, will it be surface ablation or CIRCLE-to-LASIK, and is it included?
  • What happens if suction is lost during the laser?

A clinic that answers these clearly is more reassuring than one that only quotes a price. See questions to ask your surgeon.

Who should choose which?

The VisuMax 500 (SMILE) may suit you if you:

  • Have moderate myopia, with or without mild astigmatism
  • Can stay calm and still for about half a minute
  • Want flapless surgery at a lower price

The VisuMax 800 (SMILE Pro) may suit you if you:

  • Are anxious about holding still or have had suction concerns
  • Have higher astigmatism where automated alignment aids may help
  • Want CIRCLE available for a possible future LASIK-style re-treatment

Either laser needs enough corneal thickness and a healthy cornea; neither is suitable for keratoconus. A full exam and your surgeon's judgement should decide. See top SMILE clinics in Seoul.

FAQ

Frequently asked questions

What is the difference between VisuMax 500 and 800?

The VisuMax 800 fires at 2 MHz instead of 500 kHz, cuts the SMILE lenticule in under 10 seconds instead of about 25–30, and adds automated centration, cyclotorsion and CIRCLE re-treatment tools.

Is SMILE Pro better than SMILE?

SMILE Pro is faster and has newer alignment aids, but published experience suggests similar visual results. A skilled surgeon on either laser matters more.

How much more does SMILE Pro cost in Korea?

In Seoul, SMILE costs about ₩2.2M–₩3.2M and SMILE Pro about ₩3M–₩5M for both eyes, typically around ₩4M (2026).

Is the VisuMax 800 FDA approved?

Yes. ZEISS announced US FDA approval of the VisuMax 800 with SMILE pro software in January 2024, for nearsightedness with or without astigmatism.

Is suction loss less likely with the VisuMax 800?

Shorter laser time should reduce the chance, and one surgeon reported 0.08% on the 800, but there is no controlled head-to-head study yet.
